چکیده

To quantify clinical and operative factors influencing comeal graft outcome. MeVlods Multifactorial analysis of 2242 grafts registered by United Kingdom Transplant Service from July 1987 to June 1990. Results Increased risk of graft failure with: pm operative stmmal oedema; small trechine size: difference in donor and recioient sizes areater than 0.25 mm: and mixed ccntinuous and interrupted sutures. Poorer 3month visuai acuity with: glaucoma and low visual acuity preoperatively; small trephine size; and combined vitreous surgery. Higher 5 month astigmatism with: interupted sutures. Conclusions Effects of factors were generally in the same direction for each outcome measure. Only large scale, randomised. controlled trials will provide definitive answers to preferred operative techniques.

Influence of HLA-A, HLA-B, and HLA-DR matching on rejection of random corneal grafts using corneal tissue for retrospective DNA HLA typing.

AIM To establish if coincidental HLA-A, HLA-B, and HLA-DR tissue matching is associated with a reduced likelihood of corneal graft rejection. METHODS Organ culture preserved random donor corneas were used for penetrating keratoplasty (PKP). Corneal tissue from all graft recipients and donors or blood samples from recipients after repeated transplantation were obtained in order to perform retr...

Matching for Human Leukocyte Antigens (HLA) in corneal transplantation - to do or not to do.

As many patients with severe corneal disease are not even considered as candidates for a human graft due to their high risk of rejection, it is essential to find ways to reduce the chance of rejection. One of the options is proper matching of the cornea donor and recipient for the Human Leukocyte Antigens (HLA), a subject of much debate. Currently, patients receiving their first corneal allogra...

The role of HLA-DRB1 matching in corneal grafting.

The aim of the study was to evaluate the role of HLA-DRB1 (Human Leukocyte Antigens) matching in corneal transplantation. Fifty-two patients were observed. Low-risk group consisted of 28 patients and high-risk group consisted of 24 patients. All the patients and donors were tissue typed with Polymerase Chain Reaction (PCR) on the HLA-DRB1 gen. The primary corneal disease preceding keratoplasty ...

The Predictive Value of HLA-DR Matching and Cytokine Gene Polymorphisms in Renal Allograft Acute Rejection: A Living-unrelated Donor (LURD) Study

Background: In addition to Human Leukocyte Antigens (HLA) compatibility, gene polymorphisms in cytokines might also be important in the quality of allogeneic immune response. Objective: To evaluate the influence of HLA-DR matching and a number of cytokine gene polymorphisms on acute rejection after living-unrelated donor (LURD) kidney transplantation. Methods: A total of 42 renal transplants pe...

Histocompatibility testing for keratoplasty in high-risk patients.

Forty-eight consecutive patients at high risk for corneal graft rejection were transplanted with corneas from donors selected on the basis of a negative lymphocyte crossmatch, ABO blood group compatibility, and maximized HLA-A, -B matching. Graft survival in this study group was compared to a retrospective control group of 72 consecutive high-risk transplants performed in patients receiving ran...
